چکیده

Diuretics continue to be among the commonest of all prescribed items. Until recently, the physician needed to have little concern about the dose of the agent he was choosing because firstly, the dose-response curve tended to be startlingly flat, and secondly, there was little chance of inducing an excessive diuresis, regardless of dose. Now that furosemide (Lasix) and ethacrynic acid (Edecrin) have become available, we find that one must indeed titrate the dose to the particular patient, and must appreciate for the first time the complications which can result from an excessive diuresis. Finally, with the availability of triamterene and MK870 ( Colectril, not yet available), one must appreciate the importance of the distal tubular mechanism for potassium for sodium exchange. The additive effect of these exchange inhibitors can be of considerable clinical usefulness .

Sildenafil and furosemide associated ototoxicity: consideration of drug-drug interactions, synergy, and broader clinical relevance.

Drug-induced ototoxicity, particularly those involving phosphodiesterase type 5 (PDE-5) inhibitors, is considered to be rare and to our knowledge such an adverse effect has not been reported in Canada. Here we present a case of a 77-year old man initiated on a sildenfil regimen for the treatment of pulmonary hypertension, who developed sudden bilateral hearing loss after taking sildenafil, in t...

Statins: Newer Roles Including Lipid Lowering Therapy

Statins are most effective pharmacologically agents to decrease total plasma cholesterol level by competitive inhibition of HMG-Co A reductase. Apart from the well-known LDL and cholesterol lowering effect, statins have been postulated to exert other beneficial effects also called pleotropic or non-lipid effects. Better understanding of various pleotropic effects of statins has prompted a new s...

Newer Drugs in the Treatment of Hypertension.

Rauwolfia compounds, bretylium tosylate, guanethidine, alpha-me...
